Witness of triple fatal Piedmont Cybertruck crash describes what happened

SAN FRANCISCO (KRON) -- California Highway Patrol investigators released a preliminary report revealing what caused a triple fatal Tesla Cybertruck crash in Piedmont. Four college students were trapped inside the Cybertruck after it burst into flames and three died. A witness rescued one passenger and told CHP investigators about what happened.

The CHP report states that a combination of alcohol intoxication, drug impairment, and unsafe speeds caused the driver to crash.

Soren Dixon, 19, Jack Nelson, 20, and Krysta Tsukahara, 19, died from asphyxia due to smoke inhalation and severe burns, according to an autopsy report by the Alameda County Coroner's Bureau.

A fourth college student who was a passenger, 20-year-old Jordan Miller, survived because a witness was able to break window using a tree branch and pull him out of the mangled Cybertruck.

Dixon was driving when the Cybertruck veered off Hampton Road, slammed into a concrete retaining wall and a tree, and was rapidly engulfed in flames around 3 a.m. on Nov. 27, 2024, a sheriff's coroner's investigation report states. Dixon had alcohol and cocaine in his system when he died, according to the coroner's report. His Blood Alcohol Concentration was 0.195.

Tsukahara, Nelson, and Dixon were all recent graduates of Piedmont High School.

A witness told CHP investigators that the college students attended a small party together at a friend's house. After the party, Dixon was attempting to drive his friends to Miller's house when the crash happened, the report states. The witness, who was following behind the group in a second vehicle, briefly lost sight of the Cybertruck just before it crashed.

The witness broke a front passenger window by hitting it 10-15 times with a tree branch, pulled Miller out, and "was unable to continue assisting others out of (the Cybertruck) due to the severity of the fire," CHP investigators wrote.

The witness said, "I saw a car on fire wedged between a wall and a tree. I instantly knew by the tail it was the Cybertruck. (Miller) was barely conscious. He had his seatbelt on and I couldn't reach it, so I scream at him to undo it. Eventually he was able to unbuckle. I was able to pull him up and out of the window."

The witness continued, "I went back to the broken window and yelled for them to try and get out at this window. Krysta tried to come up, sticking her head from the back. I grabbed her arm to try and pull her towards me but she retreated because of the fire."

The witness told CHP officers, "I then grabbed the stick again and began to break the right rear window. I was able to break and pull the window from its frame but once I got in, the fire had made its way throughout the entire car. It was at this point that the first two squad cars arrived. I immediately ran to them yelling there are people in there."

Police officers and firefighters attempted to vain to extinguish the flames before the three victims were killed. The CHP report states that the Cybertruck sustained major damages from the force of the collision, and flames spread through both the driver and passenger compartments.

The name of the Cybertruck's owner is redacted in the CHP's report.

A full investigation by the CHP’s Golden Gate Division Multidisciplinary Accident Investigation Team will be completed later this year.
