At least four others were wounded in the shooting rampage in the western town of Cetinje on Wednesday that followed a bar brawl, officials said.
The shooter, identified as 45-year-old Aco Martinović, killed the owner of the bar, the bar owner's children and his own family members, Interior Minister Danilo Šaranović said.
Martinovic was on the run after opening fire on Wednesday afternoon at a restaurant in Cetinje, a small town located 38 km (23.6 miles) west of Podgorica, the Montenegrin capital, where he killed four people.
The shooter then moved on to three other locations, killing at least six more people, including two children, police said. Four other people suffered life-threatening injuries.
Police said Martinovic had a history of illegal weapons possession.
Late on Wednesday, police director Lazar Scepanovic said the suspect was thought to have been drinking heavily before the shooting. Montenegrin Prime Minister Milojko Spajic said there had been a brawl before shots were fired.
Writing on social media, the prime minister said: "This senseless act has caused immeasurable sadness and bitterness in each of us. There are no words of comfort."
Milojko Spajic said the country's security council would "urgently consider all options" to protect the public, including a complete ban on the possession of weapons.
Mass shootings are comparatively rare in the small Balkan nation.
The report said he went home to get his gun and came back to the bar where he opened fire and killed and wounded several people. He then went to another site where he killed the bar owner’s children and a woman, the report added.
