The 34-year-old was attempting to grab an offensive rebound from a missed three-pointer when he suddenly collapsed and quickly reached for his left calf and Achilles tendon.
The next dead ball saw Lillard being helped to his feet by teammate Kyle Kuzma before hopping his way to the bench with the assistance of coach Doc Rivers.
There were fears that the nine-time NBA All-Star Lillard suffered a torn Achilles.
Before the first half, the Bucks announced that Lillard wouldn’t return due to a “lower leg injury.”
A little after the team’s update, TNT Sports’ Chris Haynes reported that there’s fear Lilard suffered a torn left Achilles tendon.
Lillard will undergo an MRI on Monday.
“Just being honest, it’s not very promising,” Rivers said after the Bucks’ 129-103 loss on Sunday night.
The Bucks were down by just three when Lillard went down with the injury.
The deficit grew to 11 by halftime, and Rivers admitted the team was deflated after Lillard’s serious injury.
“Dame’s one of them special ones,” Bucks guard Kevin Porter Jr. said.
“He’s very much needed, very much impactful in every way.
“Seeing him go down and not be able to return, it’s definitely defeating.”
The Bucks now trail 3-1 in the series.
Lillard missed the team’s final 14 regular-season games due to deep vein thrombosis in his right leg.

But Lillard hasn’t looked like his normal self, averaging just 10.5 points and 6.0 assists on a disappointing 24 percent shooting percentage in the first two games.
The Giannis Antetokounmpo-led Bucks will prepare to be without Lillard as they try to avoid a third consecutive first-round playoff exit.
Game 5 will be on Tuesday in Indianapolis.
